Annual Food Establishment Permit fees are as follows:

1 to 6 Employees $200.00               7 to 15 Employees $250.00

16 to 25 Employees $300.00          26 to 35 Employees $350.00

36 to 50 Employees $400.00

Licensed Day Care Facilities Permit fees are as follows:

1 to 20 Children $150.00                21 to 30 Children $175.00

31 to 50 Children $200.00             51 to 75 Children $225.00

76 to 100 Children $250.00

Annual Food Establishment Permits expire on December 31st of the calendar year of its issuance.     Late fee (if the permit is not renewed prior to the expiration date) $50.00

The reinstatement fee for a suspended permits is $75.00.

Temporary Food Establishment Permit fees are $40.00, valid for one event and not to exceed 14 days.  An additional late fee of $40.00 will be assessed if the permit is not received prior to the opening of the event.
